I/We hereby authorize you to obtain further information on my/our credit and employment history and such source is hereby authorized to disclose any Credit Bureau and together credit grantors and information about my/our credit history. I/we agree to indemnify you jointly and Severally against any and all claims in damages or otherwise arising from disclosure on your part.

If the loan is granted, I hereby agree to comply with all the terms, bye-laws, and regulations of the society, now in force or which may hereafter be adopted. The statements herein are made for the purpose of obtaining the loan and are true and correct to the best of my knowledge and belief.

Note: Once a credit report is generated you would be charged a fee of $100.00 .

I/We understand that upon the maturity of this request, I am to resume full loan payments. I/We understand that the loan schedule will be adjusted accordingly to account for any past due principal payments. 

I/We declare that all information herein given is true, accurate, and complete to the best of my knowledge and was provided for obtaining the non-payment of my/our principal loan payment(s)

I hereby apply for the aforementioned Statement from Bethel Credit Union Co-operative Society Limited and agree to abide by the bye laws and rules or any amendment thereof the Credit Union. I declare that I am not engaged in money laundering, terrorist financing, fraud, drug trafficking, identity theft, or any other criminal or illicit activities. Authorization and consent is given to Bethel Credit Union to obtain information about me and verify the submitted information.

I hereby request that my account be transferred to the aforementioned Branch of the Bethel Credit Union with immediate effect. I declare that I am not engaged in money laundering, terrorist financing, fraud, drug trafficking, identity theft, or any other criminal or illicit activities. Authorization is given to Bethel Credit Union to obtain information about me and verify the submitted information.

I hereby authorize my employer to deduct and transmit the sum mentioned above to Bethel Credit Union Co-operative
Society Limited. No changes should be made to this agreement unless a request is made in writing and singed by
Bethel Credit Union Co-operative Society Limited, a witness and myself.
